High Reliability Organization Transformation and the Veterans Health Administration 

The Veterans Health Administration (VHA) announced that it has begun efforts to shift the U.S. Department of Veterans Affairs (VA) medical system into a high reliability organization. This shift has begun with the selection of 18 VA medical facilities across the country who will begin their high reliability journeys in the coming year. According to the Agency for Healthcare Research and Quality, high reliability organizations are “organizations that operate in complex, high-hazard domains for extended periods without serious accidents or catastrophic failures. High reliability organizations use systems thinking to evaluate and design for safety, but they are keenly aware that safety is an emergent, rather than a static, property.” Visionary Consulting Partners, LLC (Visionary) acknowledges the criticality of implementing this model across sites, as it will foster a shift in organizational culture to that of safety, harm reduction, and continuous process improvement.
